چکیده

Liver transplantation, whether living donor (LDLT) or deceased donor (DDLT), is currently the treatment of choice for patients with advanced liver disease. While initially the focus was on acceptable short-term survival, currently the efforts are aimed at improving longterm prognosis. Thus, focus is now on the quality of life after liver transplantation, as well as prediction and management of conditions related to morbidity and mortality in long-term survivors. Renal dysfunction is an important problem in this scenario. Both acute (ARD) and chronic renal dysfunctions (CRD) develop frequently after liver transplantation and can seriously jeopardize postoperative patient survival.

پیوند همزمان کبد و کلیه در بیمار مبتلا به نارسایی کبدی و کلیوی در یک بیمار 18 ساله

Introduction: Over the years, renal insufficiency caused by liver disorders was an obstacle to liver transplantation in these patients. After presenting a system for the evaluation of various stages of liver disease, simultaneous transplantation of the liver and kidney is considered an appropriate therapeutic treatment in patients who are at the end stage of the liver disease and have secondary...

Interfaces between medical specialties Renal dysfunction associated with liver transplantation

It has been known for some time that a variety of liver diseases affect kidney function, but renal dysfunction associated with orthotopic liver transplantation has received scant attention. Although the mechanisms mediating these abnormalities are incompletely defined, advances in the understanding of renal pathophysiology after liver transplantation have made it possible to develop new treatme...

Differential Simultaneous Liver and Kidney Transplant Benefit Based on Severity of Liver Damage at the Time of Transplantation

BACKGROUND We evaluated the concept of whether liver failure patients with a superimposed kidney injury receiving a simultaneous liver and kidney transplant (SLKT) have similar outcomes compared to patients with liver failure without a kidney injury receiving a liver transplantation (LT) alone. METHODS Using data from the United Network of Organ Sharing (UNOS) database, patients were divided ...

Increased Expression of Toll-Like Receptors 2 and 4 in Renal Transplant Recipients that Develop Allograft Dysfunction: A Cohort Study

Background: The incidence of ischemic reperfusion injury (IRI) in early phase post-transplantation and activation of toll-like receptor (TLR-2) and TLR-4 remarkably impact the outcome of a renal allograft. Objective: To investigate whether the expression of TLRs in peripheral blood mononuclear cells (PBMCs) can predict the clinical outcome of kidney allografts. Methods: We obtained blood sample...

Effects of Pretreatment with Pentoxyfilline and N-acetylcysteine on Renal Injury Following Induction of Liver Ischemia-Reperfusion

Background & Aims: Liver ischemia-reperfusion (IR) is one of the common consequences of liver surgery, especially during liver transplantation which results in organ dysfunction. Acute hepatic injury causes systematic inflammatory responses which may finally lead to functional disturbances in remote organs such as heart, lungs and kidneys. In this study, the effects of a potent inhibitor of inf...

Predictors of renal recovery in patients with pre-orthotopic liver transplant (OLT) renal dysfunction

BACKGROUND Renal dysfunction occurs commonly in patients awaiting orthotopic liver transplantation (OLT) for end-stage liver disease. The use of simultaneous liver-kidney transplantation has increased in the MELD scoring era. As patients may recover renal function after OLT, identifying factors predictive of renal recovery is a critical issue, especially given the scarcity of available organs. ...
